About the case

Assess a Patient Presenting with Headaches and Elevated Blood Pressure

Your patient is presenting with a one-month history of frequent, worsening headaches and a blood pressure reading of 168/86 mmHg taken recently at a pharmacy. The headaches are dull, throbbing, and constant, rated 5 out of 10, lasting about two hours each episode and occurring five times per week. The current headache episode began one hour ago. The patient's medical history includes Type 2 Diabetes managed with glyburide 10 mg daily, a family history of hypertension, elevated cholesterol, and gout on the paternal side, and mild ankle swelling at the end of workdays.

The patient reports high work-related stress over the past year as an aggravating factor. Red flag symptoms including sudden onset, neurological deficits, vision changes, chest pain, and shortness of breath are all absent. As the clinician, your role is to conduct a thorough history, identify hypertension-related and headache-specific risk factors, rule out dangerous causes, and guide appropriate next steps.

Learning Objectives
  • Conducting a structured headache history including onset, quality, location, severity, timing, and triggers
  • Identifying hypertension risk factors including family history, comorbidities, stress, and lifestyle
  • Ruling out red flag symptoms such as sudden onset, neurological deficits, vision changes, and chest pain
  • Reviewing current medications, social history, tobacco and alcohol use, and family medical background
  • Communicating findings with empathy and guiding the patient toward next steps for blood pressure management
